Summary
The Shoalwater Bay Wellness Center is excited to announce that we have an opening for a 32hr a week Diabetes Educator. We are looking for a team player with the ability to handle multiple work tasks and communicate effectively. The Diabetes Educator will maximize the use of resources to enable chronic disease management for patients with diabetes. They will focus on diabetes care, diabetes prevention, and diabetes education. They will work in collaboration with clinical providers to develop education and medical treatment plans for patients with or at risk for diabetes. This includes working with prescribing providers to address medication adjustments as necessary. The Diabetes Educator provides education related to the diabetes disease process, medications, nutrition, physical activity, foot care, acute and chronic complications, and blood glucose monitoring. They provide instruction on the use of home glucose monitors, including continuous glucose monitors. They also provide instruction on injecting and managing insulin.
32hrs per week Non-exempt
RN Wage $37/hourly
MA/LPN wage range- $27-35/hourly DOQ
Job Description
Essential Duties and Responsibilities
The Diabetes Educator will be responsible for the tribe’s SDPI (Special Diabetes Program for Indians) grant which includes but is not limited to:
- Assisting the grant writer with the grant application, budget submission.
- Planning and implementing the activities of the grant.
- Submitting required grant data and reports in a timely manner for compliance
- Attending meetings and trainings as required for grant programs
- Conducting a yearly audit of diabetes care and outcomes for the AI/AN population.
- Conducting community outreach events and collaborating with other departments and grant programs to raise awareness about diabetes and diabetes management within the community
- Writing articles for the local newsletter regarding grant activities
Other responsibilities include:
- Assisting the Nutritionist with classes, webinars, and nutrition education activities
- Recording all pertinent information in patient(s) clinical electronic health records
- Performing all duties in accordance with State regulations, while adhering to facility policy and procedures.
Qualifications
- Bachelors of Science degree in nursing (BSN) or an Associate’s degree in nursing (ADN) from College or technical school; and two to four years related on the job experience and/or training; or equivalent combination of education and experience.
- Certification as a Certified Diabetes Care and Education Specialist (CDCES) preferred or the ability to obtain such.
- LPN or an MA will be considered if minimum qualifications in Diabetes education are met.
- 2+ years of experience REQUIRED.
- EPIC knowledge preferred.
Equivalent experience will not be accepted in place of required education.
